Landlord Didn't Submit Proper DOB Form

Landlord applied for MCI rent increases based on the installation of a boiler/burner. The DRA ruled against landlord because landlord didn't submit a DOB form 16a regarding the installation. Landlord appealed and lost. Landlord's submission of a DOB on-site inspection form wasn't an adequate substitute for the missing form.
