Landlord Used Wrong Deregulation Form

Landlord applied in 1995 for high-rent/high-income deregulation of tenant's apartment. The DRA ruled against landlord and dismissed the petition. Landlord's petition was incomplete and because it was filed after June 15, it wasn't returned to landlord for amendment by June 30. Landlord appealed, claiming the form was filed properly. The DHCR ruled against landlord. Landlord mistakenly used a 1994 deregulation petition form for 1995. In 1995 the form required landlord to submit to the DHCR the Income Certification Form and proof of service to tenant. Landlord didn't do this.
